How to fill in the form

The form is available after logging in by the verification code which will be sent to the email address provided.

Cells marked with asterisk (*) are mandatory. If you do not fill them, the form will notify you by a notice and red exclamation mark (!) placed before the incorrectly filled cell.

Fill in the form as follows:

1. Contact details of notifier

   Marketing authorisation holder (MAH) is a notifier of export. Insert first name, surname and address of business place if different from MAH´s permanent residence address in case of a natural person - name or business name and address of MAH´s residence in case of a legal entity.

2. Contact details of wholesale distribution authorisation holder (WDAH) with power of attorney

   Insert first name, surname and address of business place if different from WDAH´s permanent residence address in case of a natural person - name or business name and address of WDAH´s residence in case of a legal entity. Only WDAH authorised by your power of attorney can export a medicinal product.

3. Medicinal product

   Insert data on all exported medicinal products. To add another medicinal product, choose ADD ANOTHER MEDICINAL PRODUCT.

   Name of medicinal product: After writing first three letters, the database of registered medicinal products will be activated. Choose a medicinal product according to SIDC code, name, strength and package size,

   Batch number: Batch number of medicinal product,

   Number of packages: Number of packages of medicinal product,

   Prize: Unit prize of package in officially determined prize,

   Total prize: Total prize of exported medicinal product,

   State: State where medicinal product was exported to. After inserting the first letter, the list of states will be activated,

   Date: Date of medicinal product export. You can use either a coma or dash to separate numbers,

   Reason: Justification of medicinal product export.

4. To save and send form

   You can either SAVE or SENDa form.

   SAVED form will be not processed by SIDC and you can adjust it after logging in by using the same email address as before.

   SENT form will be processed by SIDC and published on its website. You will be informed once the form is sent successfully with the possibility to open the sent form. You will receive an automatically sent confirmation to your email address.

5. Power of Attorney

   Send the Power of Attorney as a pdf document to the e-mail address vyvoz@sukl.sk within 7 days after the day of export.

6. Publishing of notification on web page of SIDC

   After checking the correctness of completion of the notification and the power of attorney, the export notification will be published immediately on the web page of SIDC in the section „INFORMÁCIE PRE VEREJNOSŤ – Zoznam vyvezených liekov“ – NOTE: This part is available only in Slovak version of the web page.
